Community Health Educator

Working as a key member of the Health & Wellness Team, the Community Health Educator/Coordinator will be
responsible for the coordination and implementation of preventative health and disease management
education programs to expectant and parenting women and their children in designated communities within
Maricopa County.

Core Responsibilities:
Coordination:

  • Coordinate and schedule health education programming in collaboration with site partners in various valley locations.
  • Actively participate in community outreach efforts and relationship building to establish strong relationships
    with current and new community partners.
  • Coordinate and support health and wellness coaching to achieve optimal health outcomes.
  • Assist with the coordination of special events for community participants.
    Facilitation
  • Implement preventative health and disease management education (e.g. nutrition, diabetes, chronic
    disease, oral health and much more) to promote optimal wellness by following best practices and curricula
    designed for achieving established outcomes.
  • Assist with the collection and management of metrics and program outcome evaluation by administering
    participant surveys and inputting data for data analysis.
  • Collect impact stories from community participants on the effectiveness of the implemented health
    programming.

Additional Responsibilities:

  • Performs all functions and activities within the guidelines and philosophy set forth in Advance’s strategic plan, policies, mission, goals, and vision.
  • Work collaboratively with Advance’s team members and volunteers.
  • Other related duties as assigned by the Health and Wellness Program Director.

Minimum Qualifications:

  • High School Diploma or Community Health Worker Certificate AND three (3) years related experience in
    community development and social services; OR any equivalent combination of education and/or
    experience from which comparable knowledge, skills, and abilities have been achieved.
  • Fluency in both English and Spanish.
  • Experience and competence working with women and children.
  • Cultural knowledge and awareness in understanding under resourced communities within the Phoenix
    Metropolitan area.
  • Organized, self-motivated and self-directed team player with the ability to adapt to diverse
    environments, programming, people, and situations.
  • Ability to maintain effective working relationships and communicate (written and oral communication)
    in a pleasant, positive manner with community members, community partners, and coworkers.
  • Must be able to walk, stoop, kneel, reach, crouch and lift a minimum of 30 pounds with or without
    accommodations.
  • Must have reliable transportation with valid driver’s license.
  • Food Handlers Card- must obtain or be willing to obtain.
  • CPR and First Aid Certification- must obtain or be willing to obtain.
  • Proof of completed Covid-19 vaccination and approved boosters.
    Security Clearance Requirements
  • Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.

office equipment, avoiding trips and falls, observing fire regulations, etc. Typically, the employee may sit

as papers, books, small parts, and occasional lifting of boxes or containers not to exceed 30 pounds, driving an
automobile, etc.
